Office Policies 


  • In accordance with HIPAA privacy regulations, we do not fax medical records to unsecure, unattended lines, please arrange to pick up records.  
  • No cell phone or recording device use is permitted in the office in accordance with HIPAA regulations. Please turn your cell phone off while you are at the office.
  • All medical forms take 5-7 business days to process. Please make sure that all forms being turned in are fully completed.
  • You may receive secure message reminders and link to our Facebook Page


  • Please give us 24 hours notice for any cancellations. We are holding those times to accommodate your children and we would like to offer that time to another child.

  • Lyndon Pediatrics accepts cash, check, and the following major credit cards: Visa, Mastercard, American Express.
  • Deductible charges and Co-payments are due at your appointment. The amount of your co-payment will depend on your insurance company, deductible payments at a minimum will be $25 with the rest billed to your insurance.
  • If you have no insurance, full payment is due at your appointment.
  • The adult attending to the patient at the visit is responsible for payment at the time of the appointment.  PLEASE ARRANGE THAT THIS ADULT BRINGS PAYMENT TO THE VISIT.


  • Please have all necessary forms filled out before your examination. Please arrive 5 minutes early to your appointment to fill out the necessary forms, or download the forms you need here. 
  • There will be a $5.00 charge for any form that needs to be re-done or completed.
  • If you are coming in for a visit with one child, please call in advance if you want another child seen.


  • New York State law requires prescriptions to be electronically sent to the pharmacy, INCLUDING CONTROLLED SUBSTANCES such as ADD medication.  Please allow up to 5 to 7 business days to prescribe controlled substances as they require special coding.
  • Call at least one week before you are due to run out of your medication.
  • Be prepared to give the office staff a complete request for the medication. This includes the medication name, prescription number, dosage, frequency, etc.


  • Lyndon Pediatric Associates follows the immunization schedule recommended by the American Academy of Pediatrics.
  • We DO NOT skip immunizations. We do not accept patients who refuse vaccines.
  • A parent or legal guardian must be present for children to receive immunizations. If no parent or guardian is present, child must have written permission from their parent or guardian.
  • An updated immunization form will be presented to patients at every wellness exam as feasibly possible.
